WHO WE ARE

The Population Health Leaders Network (PHL Network) connects healthcare leaders to promote and accelerate the adoption of innovative population health models. We represent national health systems dedicated to driving change, collaborating around best practices, assessing and guiding leading-edge pilot opportunities, and sharing best practice results to support healthcare delivery.

We recognize that a cost imbalance exists today, as 5% of the population accounts for 50% of all health spending. Of the modifiable contributors to health outcomes, only 10-20% are associated with medical care. Recognizing this need to shift the way we deliver care to manage costs and look beyond traditional medical care, we have created a forum to identify, assess, test, and share strategies to manage the rising costs of care and improve patient outcomes.1,2

Together this network identifies and works to promote forward-thinking models by working across the healthcare ecosystem to endorse new concepts. The Population Health Network is powered by The Kinetix Group and headquartered in New York City.


OUR MISSION

To identify and accelerate new approaches to care delivery nationwide through testing and vetting of specific service offerings with the help of our multidisciplinary team of experts.
